Cord clamping HELP :)

Hi,

Had a heated discussion with OB lastnight.
He is a big believer of early clamping (immediatly) syntocin and controlled traction for placental removal (or something like that)

My feelings
1) dont care either way about the inj.
2) wanted plactenta to finsh pumping before clamping
3) traction / manual removal of placenta is it necessary????
4) wanted to BF to stimulate 3rd stage
5) didnt want time limits

His view point
1) Best practice statistics demonstrate it is the safest way to avoid post partum heamorrhage
2) agrees that BF and the nautral process will stimulate placental detactment.
3) 'I will be as flexiable as youlike with most things, but i believe that this compromises your safety to will not change my view'

He will agree to delaying clamping my 5 minutes...
Will not negotiate on other aspects of this proceedure.

To be fair - he has a low intervention rate, is happy not to have any medical intervention unless apsolutly necessary - i told him that i would prefer him just to watch and he was cool with that... so he will accomodate most things (he is also encouraging of me hypnobirthing and has others that are having water births)

My big question is : how long on average does the placent pump for (he couldnt answer this as he has NEVER waited). Is 5 minutes adequate?
Spoke with DH and he feels that OB decision is OK as he has been a bit flexable and that my safety is # 1

I am pretty sure that 5 min would be considered delayed. From memory an article i read said anything after 2min was considered delayed cord clamping...... maybe it was 1 min..... sorry bit vague!

it took 40 minutes for me to deliver my placenta not sure how long it took to syop pulsating!
i dont think you will get the low intervention 3rd stage you want with this ob.

Me either - dicussed everything else with him early and felt that i have the right OB as he was happy to do nothing unless medical emerg etc all the things that i wanted.
i never thought to ask him about the cord thou!

Lesson learnt although not feeling as bad about it now

From what i have read - to delay by two mins is to receive 80% of the blood/iron/nutrient stores from the placenta.
From there it can vary tp up to twenty mins to stop pulsating. Some doctors/hospitals dont like to delay clamping because they think it can cause jaundice in the baby (i think) - BUT the extra value your bub would get out of delaying clamping is much better for a couple of hours spent under phototherapy.

Early clamping (immediatly) syntocin and controlled traction are still interventions in the normal birth process.

However, if he's agreed to 5 mins (and wont budge) then this may be your only option. I suggest you have someone else with a watch though because he might not actually wait the 5 mins unless someone else is timing it.

Oh no - we will be timing it!!!
He will be fine with that and probably laugh - he is a really good OB and i have no real reason to complain, this just took me by suprise!

Funny how he says its best practice, the studies (and world health organisation) are saying otherwise!!! More likely to bleed if you are induced/augmented with a synto drip. Not likely with a normal vaginal birth.
